The footer is set to 'floating'. But the form is fixed. That is why footer is not moved.
You will need to set the footer to 'Default' or make all other content floating. For example, by using layout grids/floating layers.

You can set the position of the dialog in the properties.
Currently it is set to:
Horizontal: Center
Vertical: Center

I'm sorry, it is not possible to convert a Excel xlsx spreadsheet to a form.
That would require programming, it cannot be done 'visually'.

The background will only show up if the flex grid has content.
Otherwise the height of the grid will be zero (unless the rows have a fixed height).

First note that the implementation of your breakpoints is incorrect:
the default view should be the largest view.

Unfortunately the modal layer (jQuery UI ) does not support breakpoints.
However, if you set it to Bootstrap then it may work.
Or alternatively you can use the jQuery Dialog.
