You can do this visually by creating a standard form. The PayPal buttons only provide 'fixed' functionality.

Hold down the SHIFT key while clicking 'Export to HTML'. Note however that this will not publish your website. It will only export HTML.How can I export a page to an html format directly on my pc as I was able before?

Are you sure the 'documentready' section of your extension is empty (no spaces, line break etc)?
It seems to works correct for me. $(document).ready will only be generated if there is code in this section.

Note that older versions (7.x, 6x, 5,x 4.x) are no longer updated because these versions were built with a different programming environment that is no longer supported by the supplier.

When the option 'Require Shipping Information' is checked the customer is prompted to include a shipping address (according to the PayPal documentation).
