Submit Forms only saving first record

Hi,
I have just setup a form which works with both SQL and csv tables but will only save the first record.
There are no error messages when submitting. I am sure there must be a simple explanation.

To be able to help you, I need to see what you have done.
Do you have a demo project?

Did you check the server log for errors?

Hi,
Thanks for the reply, not to sure what you want as it is only the form wizard nothing else on the page as I am just testing at the moment.
Can you tell me how to attach an image or wbs file

Sorry, that does not help. I need the project file.

Also, did you check the logs on the server for errors?

Note: your form needs a success and error page!
