How to use?

1. Create a form as you usually do.
2. Once you have your form ready, drag and drop an instance within the form.
3. Open the properties dialog and setup the appearance according to your design scheme.
4. Preview or publish.

Nice extension; simple but effective. What does it cost? I clicked on the PayPal link at the top of the form and it took me to a different link.
