Does it exist any extensions that I can use?

There is no extension for this because a survey/poll-system is a script that needs to be installed on the server.

You can add any external scripts using the HTML Object see url below along with the File Publisher Object if required. Please note when using external scripts it is understood that you yourself have knowledge to install them.
