Hi Guys

I am trying t create a form where once a check box is ticked it shows another description with another text box.

For example name1 and checkbox1 and name2 and checkbox2

name2 and checkbox2 are not visible they only show when checkbox 1 is ticked.

I have managed to do this but when i select checkbox1 it shows name2 and checkbox2 but i need the checkbox1 to be ticked ?
