You can also use the likes of a .htaccess to prevent a range of ip addresses the downside of this is that you can also block legitimate users from using the form to make contact or even access the site or sites.

When I publish a website, all the images go to the images folder except the .jpeg files ... that image goes to the root folder with all the html/php files.

You need to add more content to your page. As it is, the menu is creating a longer page when you hover over the menu but the page is really not that long. Add an object way down the page to increase the actual page height beyond the browser height and your menu works.

I'm sorry, there is no standard solution for this. This will require a custom script.
The issue is that if the value is empty (unchecked) then the browser will not sent the value to the server. So, the variable will not exist in that case.

Yes, I have. The field is already set to $name, which matches the field of the applicant. That has no affect on how the form arrives to me. It still shows the email came from an email address instead of a name.

The CC stands for carbon copy harking back to the days were you would use carbon paper to copy a letter to send to multiple persons.
When used in emails it allows someone other or others than the primary recipient to receive a copy of the email.
It also means the recipient in both the To field ...

The "Input Field Name" is completed with "email". Shouldn't that be the email address of the applicant, as "email" is the field name for the applicant.

The form, when submitted, goes to the error.html page on the website. It's not an error message, it's an error page.
It is supposed to go to the success page.
Did you try the form at your end? If so, where did it land when you hit submit?
