GoDaddy Hosting and Forms Uhggggg Im in over my head

Have a form that I'm trying to create for a webpage.

I've had WYSIWYG since single digit releases but was never a power user.

Now on a new Rev 17 ( last one was 11" maybe 14? ) here I am struggling with a form for the first time.

I don't know where to start or where to put the files in GoDaddy. Using FileZilla to ftp my work.

I have a page at http://www.lakecountrysanta.com/c_contact.php

The form is showing up but doesn't do anything.

It also does not show the date picker that I tried to incorporate at all.

The date picker is not displayed because your website has missing files.
For example
jquery-1.12.4.min.js
jquery-ui.min.js

Make sure that you publish/upload all files.

The form seems to be working. The submit button sends the data to the server.

Thank you.

After composing the original post, I poked around and found a different tutorial which was much more helpful than the help section associated with the software itself.

I am still having issues receiving the email from the primary target but added a CC to the email option using another email address and it is at least hitting that email.

I have loved this software from the first time I used it. I just wish that I was better at designing and using the correct options.
